How do I correct my personal health information?

If the demographic information, e.g., name, address, phone number, family physician etc., requires updating, you should contact the Registration staff at the clinic/department you are/ or will be attending.

If you are aware that any other information in your patient health record is incorrect, or needs to be amended, contact the Patient Records Department. Your request for correction form will be reviewed by the author of the record. By law, the hospital/health care provider are not required to correct a professional opinion or observation made in good faith about a patient.
