We offer diabetes education, follow-up, and telephone consultation to those who are newly and previously diagnosed with:

• Diabetes
• Impaired glucose tolerance
• Impaired fasting glucose

Our health care team is composed of dietitians, nurse educators, a pharmacist and a social worker.
We offer

Group Programs*
• Type 2 diabetes
• Pre-Diabetes
• Impaired Glucose Tolerance (IGT)
• Impaired Fasting Glucose (IFG)
• Glucose Meter Class
• Heart Healthy
• Carbohydrate Counting

Individually
• Diet Assessment and Care Planning
• Insulin Pump Therapy (CSII-requires referral from endocrinologist)
• Foot screening
• Insulin Adjustment (requires permission from doctor)
• Insulin Pen Instruction (requires referral from doctor)
• Continuous Blood Glucose Monitoring (requires referral from doctor)

*We offer group program content individually if you are unable to attend a group program.

We accept referrals from health care professionals or the person living with diabetes.

• We download glucose meter data.
• We have a free library with computers, internet access, videos, and information on self-care, community services and other diabetes related topics. Please come and visit us!
• We offer telephone advice to people with diabetes, their families and the community.
• Our practice is based on current Clinical Practice Guidelines for the Management of Diabetes in Canada (CDA, CMA)

Our hours are Monday to Friday 7:30 am to 3:30 pm
