The Diabetes Education & Management Centre is made up of an adult program and a paediatric program. We accept referrals from health care professionals or the person living with diabetes. To find out more about our individual programs please click on the links to the left.
Our Vision
The staff of the Diabetes Education & Management Centre are:
- Committed to the provision of evidence based diabetes education;
- Determined that all persons with diabetes have access to our services, allowing them to stand strong and confident in their self-management abilities, or with one hand on our shoulder as long as they require;
- Seen as the primary regional diabetes resource, and leaders in care and education in south eastern Ontario;
- Mindful of the need to keep one pragmatic foot on the ground, a respectful eye to the future, learning and growing every day.
