Cardiac Rehabilitation Centre:

A multidisciplinary team of health professionals provides individualized, comprehensive cardiac rehabilitation with graduated levels of programming to assist clients in gaining an optimal lifestyle.

Location: Hotel Dieu Hospital, Kingston

Eligibility: Individuals with established heart disease. Clients may access a variety of professional services such as individual nutritional, psychosocial counselling, exercise prescription and monitoring and education about cardiac disease.

Referrals: Need a referral from a doctor. Please direct referrals to the Cardiac Rehabilitation Centre. Patients will be screened by a multidisciplinary team to determine their suitability for the program. Waiting Period: 3 months
